Diabetes Education at Central Bayside Community Health Service (CBCHC) focuses on providing holistic, client-centred, evidence-based, comprehensive diabetes care and self-management education.
The service comprises a specialist multi-disciplinary team including credentialed diabetes nurse educators, podiatrists, dietitians and physiotherapists.
The service is easily accessible, with minimal waiting periods and availability in Parkdale, Mentone, Clarinda and Chelsea. CBCHS offers flexible models of care, including in-person and telehealth services as appropriate. An interpreter service is available as required.
Fees start at $10 and are assessed on income. Fees can be waived on application, in cases of financial hardship.

Clients are able to self-refer, however, referrals are preferably received from the patient’s GP practice complete with recent pathology and medication list, to facilitate more effective Diabetes Education.
