South Eastern Melbourne Primary Health Network Path to Impact

 

Dear colleagues,

It is with great pleasure that I announce the launch of South Eastern Melbourne Primary Health Network’s (SEMPHN) new strategic plan for the period 2023-2028, ‘Path to Impact’.

The strategy sets a new and transformative direction for SEMPHN as we foster a more equitable, person-centered, and seamless health system that positively impacts health outcomes in our region. Our vision is to create opportunities for the people in south east Melbourne to live their healthiest lives.

This plan provides a clear roadmap for our organisation, outlining how we will make a positive difference in our community, the context in which we operate, the areas of focus for our work and services, the changes we aim to achieve, and how we will add value for the community.

Developed collaboratively with our Board, Clinical and Community Council, Executive Leadership Team, and staff, the 2023-2028 strategy incorporates sector frameworks and best practice approaches to refine and strengthen our approach to creating positive change in our community and the wider health sector.

There are five strategic pillars that will guide our efforts over the next five years:

  • Population health and service demand
  • Consumer-focused healthcare
  • Vibrant primary health services
  • Innovation and system reform
  • Excellence and sustainability

We value your support and recognise that collaborating with you and your collegues will be instrumental in achieving our goals. We believe that together, we can make a real and positive impact in the south east Melbourne community and the broader health system.
